InfoPath 2007

I’m fairly new to InfoPath 2007 because I haven’t used it in awhile and like to start using in our hospital. We are using MOSS for our Intranet. I work in the Information Management Division (IMD) and would like to create a form for our customers to use when ordering equipment.

Frank, what exactly do you need help with?  You can easily make a new form template with all of those fields created and controls formatted - that's just the basic setup part.  Do you need to do anything beyond just making the fields?

I just need good reference(s) material(s) to teach me 'How-to' for InfoPath. I've been looking online and in book stores and really can't find anything. I would like to 'teach' myself how to use InfoPath.

Ah, ok.  The book that got me started was named Pro InfoPath 2007.  I think it is an excellent way to get started, because it's not very long, it gives you away to test all the concepts, and it really helps get your mind geared towards using InfoPath properly.  You can also check my blog for some cool techniques that will probably come in handy for each of the forms you build: http://claytoncobb.wordpress.com.
